The Young Adults 5K Race for Mental Health Awareness Registration Form

One registration form is required per participant.  

Event: 8:00 A.M. Runners
  8:05 A.M. Walkers

Course:  The 5K R ace for Education will begin and end at Grace Baptist Church located at 52 South Sixth 
  Mount Vernon, NY 10550.  It is a 3.1 mile route.  Water stations will be located along the route.

REGISTRATION PAYMENT: $30 Per Household/ Children & Youth - Free. Please make your payment below. If you are unable to run or walk but would like to give a donation, you can do so below as well. Please  choose the fund "YAMFAM 5K."  Your registration is only valid with payment. Please retain your e-receipt.

Waiver:  In consideration of my entry being accepted, I waive any and all claims for myself, my administrators, and my heirs against all officials, Grace Baptist Church, City of Mount Vernon, sponsors and organizations connected with the Young Adults of Grace 5K Race for Mental Health Awareness for injury or illness that may directly or indirectly result from my participation in this event.  I attest that I have full knowledge of the risks involved in this event, and I am physically fit and sufficiently trained to participate in this event.
