Transportation Ministry Request Form
If you require transportation for a Grace ministry or group activity, please complete this form. Someone will contact you within 48 hours of your request with the name of your driver and other pertinent details. Thank you for your cooperation.
Note: Only authorized individuals that have been certified and qualified by the transportation coordinator can operate church vans. The requesting ministry or group is responsible for expenses.
